What is your new patient procedure?

At our office, we fill out an intake form for potential new patients. This information is then reviewed by the providers and a callback is made based on their decision.

What should I expect at my initial appointment?

For initial appointments, the office will provide a reminder call the day before confirming the details of the appointment. We ask that you arrive 15-20 minutes ahead of the scheduled appointment time to fill out paperwork and bring your insurance card.
